PARENTAL CONSENT FORM

This form is currently only available at the track.

All minors (under the age of 18) must also have a Parental Consent Form completed for EACH event. It is required that the minor and the minor’s parent or Legal Guardian complete the form.

Bringing minor friends or family members for which you are not legally responsible - prior arrangements must be made to have this form completed (and notarized) when the parent or Legal Guardian will not be present at the event. THERE ARE NO EXCEPTIONS TO THIS RULE. The minor will not be able to gain admittance to the racing/pit under any circumstances. Suggestion: Ask for a couple copies to keep with you so they can be signed in advance. This form will be available on the ODKS website once published.
